Non-fee payable contracts are primarily experience based contracts and no fee will be payable to you by Goop Quest. These contracts are not designed to form an exclusive contractor relationship.
Fee payable contracts are primarily paid contracts and a fee will be payable to you by Goop Quest on completion of your shoot. Please note that fee payable contracts come with stricter terms and conditions due to your fee. These contracts are designed to form an exclusive contractor relationship.
I understand that the completion and submission of this form does not obligate Goop Quest to utilise me as a model and agree that Goop Quest may hold the details that I have submitted via this form on file for a period not exceeding two years from the date of submission. I agree that Goop Quest may contact me via any of the contact methods above within this time.*

I agree that by submitting this form I am in good health and know of no reason why I will be unable to film. I acknowledge that Goop Quest may utilise stroboscopic effects, smoke effects and UV effects during the shoot and know of no reason or health condition why these may not be used. I understand that whilst effects can be omitted from the shoot, it is my responsibility to inform Goop Quest of any reason where I may be adversely affected by these and agree to do so immediately if called up.*
